Important:

All persons over 18 years of age or older participating in watersports activities must read and sign the WSWC Waiver and provide it with your registration the day of your lesson program.

The parent or legal guardian must register and sign for your own minor children and other minors for which you are legally responsible. A note of permission must accompany the waiver form and child in absence of parent. If more than one adult member of the same immediate family attends, then you may complete one form “Adult Participants“.

Please print the applicable waiver form, complete all blanks and submit to instructor on the day of your lesson.
